The Central Management Team of Community Care Partners dedicates itself to supporting our 4 Affiliated Partners in serving over 5,000 MassHealth Members in partnership with 14 Accountable Care Organizations/Managed Care Organizations. The Central Management Team provides the infrastructure needed to ensure the complexities of the program are managed efficiently and effectively across all partners. The Team drives the following centralized functions: Partner and ACO relations; Strategy, Quality, Program Services, Operations, and Analytics/Reporting.
Team Members
Dr. Hannah Mecaskey Conley, Program Director
Dr. Hannah Mecaskey Conley is a licensed independent clinical social worker and behavior analyst in several New England States, focusing her work on public health and community-based services. Since joining CCP at the launch of the Community Partner program in 2018 (followed by a brief interlude working at the state between March 2023 and September 2024), Hannah has held multiple leadership roles supporting both BH and LTSS programs at CCP.
Currently serving as the Program Director of the CCP Central Management Team, Hannah oversees the day-to-day operations and oversight of stakeholder relations for both BH and LTSS contracts, serving as a subject matter expert in public health and integrated care in development of public health and training initiatives to support direct are teams.
Mike Tuplin, Director of Operations
Mike fulfills the role of the Director of Operations for Community Care Partners. Mike holds a Master’s in Business Administration with Healthcare concentration from Cambridge College (formerly the New England College of Business). Mike’s work in the human services field spans over a decade, beginning at the direct care level with Vinfen’s southeast area congregate sites and progressing through various management roles within the company.
At the start of the CP program in 2018, Mike took the role of Team Leader for CCP’s Boston area Vinfen team and helped to launch the program in partnership with Community Care Partners working with both the BH and LTSS C programs. In August of 2020, Mike joined the Community Care Partners team in his current role. Since that time, Mike has focused his efforts on operational projects aimed at streamlining internal and external communication processes and developing intake systems.
Jen Hicks, Director of Quality
Jen Hicks heads up CCP’s Quality Department. Jen started her work in the non-profit sector at the Italian Home for Children as an Assistant Behavioral Clinician as part of MassHealth’s CBHI initiative. Jen moved into the role of Operations Manager at the Italian Home for Children where she managed external relations, developed strategic plans for program expansion, oversaw the department budget, and directed staff hiring and onboarding. Joining CCP in March 2020, Jen has built Community Care Partners’ quality department from the ground up; her work encompasses the areas of quality, strategy, and risk.
