Community Care Partners (CCP) has been honored to serve MassHealth members, families, and communities across Massachusetts. We are grateful for the opportunity to support our members by connecting them to healthcare, behavioral health services, long-term supports, and community resources.
Effective June 30, 2026, Community Care Partners will no longer provide Community Partners services following MassHealth’s decision to end funding for the Community Partners program.
